Yes could be fatty liver disease or cirrhosis. Not looking good.
Good thing with liver if you stop alcohol for 1 months the liver regenerates anew and if you stop fully you healed. That is if it wasn't 80% damaged

If it is fatty Liver and the other organs function excellently .Liver can never regenerate from Cirhossis. These are scars and only solution is transplant

From a distance it looks like obesity.  Very tough opponent at his age.  He has to train his body to accept less food.  Like Pundit says elsewhere, scarcity mentality affects even our bodies.  Because we have evolved in a harsh environment, it takes more than a generation for the body to adjust to plenty.
